Identity Assessment Survey

Who Am I Really? — Discovering My True Identity in Christ

Before You Begin

Based on The Law of Life, Why Do We Get Sick? by Dr. Horst Müller, and Scripture. Read each statement prayerfully. Select the number that honestly reflects your experience — not what you think the right answer should be, but what is actually true in your daily life.

“Search me, O God, and know my heart: try me, and know my thoughts: And see if there be any wicked way in me, and lead me in the way everlasting.” — Psalm 139:23-24, KJV

Section A — The False Identity Indicators

“I Am God” — Self-Sourcing Patterns

These questions reveal where the false identity “I am God” may be operating — the inherited assumption that I am self-sufficient as a source.

1. When I face a problem, my first instinct is to solve it myself before bringing it to God.

Proverbs 3:5-6 — “Trust in the LORD with all thine heart; and lean not unto thine own understanding.”

2. I feel uncomfortable or anxious when I am not in control of a situation.

Isaiah 46:10 — “My counsel shall stand, and I will do all my pleasure.”

3. My sense of worth rises or falls based on what other people think of me.

Psalm 139:14 — “I am fearfully and wonderfully made: marvellous are thy works.”

4. I find it easier to give to others than to receive from God or people.

John 15:5 — “Without me ye can do nothing.” The branch does not give first. It receives first.

5. When I sin or fail, my first response is shame, hiding, or self-condemnation rather than running to God.

1 John 1:9 — “If we confess our sins, he is faithful and just to forgive us our sins.”

6. I tend to seek love, acceptance, and belonging primarily from people rather than from God.

Romans 8:38-39 — “Neither death, nor life... shall be able to separate us from the love of God.”

7. I measure my success by what I accomplish, produce, or accumulate.

Jeremiah 9:23-24 — “Let not the wise man glory in his wisdom... but let him that glorieth glory in this, that he understandeth and knoweth me.”

8. I struggle to rest — physically, mentally, or spiritually — without feeling guilty or unproductive.

Matthew 11:28 — “Come unto me, all ye that labour and are heavy laden, and I will give you rest.”

Section B — The True Identity Indicators

“I Am a Child of God” — Creature Patterns

These questions reveal where the true identity is operating — the restored understanding that I am a channel, a dependent creature, drawing life from God as the appointed source.

9. When I face a problem, my first instinct is to pray and wait on God before acting.

Philippians 4:6 — “In every thing by prayer and supplication with thanksgiving let your requests be made known unto God.”

10. I experience genuine peace even in situations I cannot control.

Isaiah 26:3 — “Thou wilt keep him in perfect peace, whose mind is stayed on thee.”

11. My sense of worth remains stable because it is rooted in who God says I am, not in performance or approval.

John 1:12 — “As many as received him, to them gave he power to become the sons of God.”

12. I genuinely receive from God first — love, strength, wisdom — before trying to give to others.

1 John 4:19 — “We love him, because he first loved us.”

13. When I sin or fail, I run quickly to God in confession rather than hiding or trying to fix it myself.

Hebrews 4:16 — “Let us therefore come boldly unto the throne of grace, that we may obtain mercy.”

14. I experience God’s love as a daily personal reality, not just a theological concept.

Romans 8:16 — “The Spirit itself beareth witness with our spirit, that we are the children of God.”

15. I can celebrate others’ success without envy or comparison because my identity is secure in God.

Proverbs 14:30 — “A sound heart is the life of the flesh: but envy the rottenness of the bones.”

16. I experience genuine rest — body, mind, and spirit — because I trust God as my provider and source.

Psalm 23:1-2 — “The LORD is my shepherd; I shall not want. He maketh me to lie down in green pastures.”

Section C — The Health Connection

Physical and Emotional Symptoms as Identity Indicators

Dr. Müller’s Why Do We Get Sick? establishes that unmet spiritual needs produce physical symptoms. These questions help identify where the body may be signaling an identity problem.

17. I frequently experience anxiety, worry, or a sense of dread about the future.

Matthew 6:34 — “Take therefore no thought for the morrow.” Anxiety is a signal that control — an “I am God” function — is being grasped.

18. I struggle with persistent physical symptoms that doctors cannot fully explain.

Proverbs 17:22 — “A merry heart doeth good like a medicine: but a broken spirit drieth the bones.”

19. I frequently feel emotionally empty, unloved, or unaccepted — even when surrounded by people.

Psalm 27:10 — “When my father and my mother forsake me, then the LORD will take me up.”

20. I find it difficult to forgive others or let go of past hurts.

Ephesians 4:32 — “And be ye kind one to another, tenderhearted, forgiving one another, even as God for Christ’s sake hath forgiven you.”

After completing this survey, sit quietly for five minutes and ask God one question: “Lord, which one false identity pattern do You want to address in me today?” Then bring it to Him in the šûb of Hosea 14:1 — “O Israel, return unto the LORD thy God.” One step. One surrender. One truth replacing one lie. That is how the identity shifts. Not all at once. But genuinely. And permanently.
